1. Resident Population Assessment: - This involves evaluating the characteristics and needs of the resident population, including their medical conditions, care requirements, and special considerations like language, culture, or religious preferences.

2. Resources Assessment: - Nursing homes must assess the availability and adequacy of resources, including staffing levels, equipment, and supplies, to meet the needs of their residents.

3. Personnel Assessment: - Assessing the qualifications, training, and competencies of the facility's staff to ensure they are capable of providing appropriate care and services to residents.

4. Services Assessment: - Evaluating the range and quality of services provided to residents, including medical care, therapy services, social activities, and dietary services.

5. Resident Choice and Quality of Life Assessment: - Assessing how the facility supports resident choices and maintains or improves their quality of life. This includes assessing the provision of person-centered care and individualized service plans.

6. Quality Assurance and Performance Improvement (QAPI) Assessment: - Evaluating the nursing home's quality assurance program, which includes ongoing performance improvement activities designed to enhance the quality of care and services provided.

7. Cultural Competency Assessment: - Assessing how the nursing home accommodates the cultural and language needs of residents.

8. Emergency Preparedness Assessment: - Ensuring the nursing home is adequately prepared for emergencies and disasters, with plans in place to protect the safety and well-being of residents.

9. Compliance with Regulations Assessment: - Confirming that the facility is in compliance with federal and state regulations related to nursing home care and services.

10. Physical Environment Assessment: - Evaluating the facility's physical environment to ensure it is safe, clean, and well-maintained, with attention to infection control and other environmental factors.

11. Governance and Leadership Assessment: - Assessing the leadership, governance, and management of the facility to ensure proper oversight and accountability. The CMS Facility Assessment is a dynamic process that nursing homes are required to perform regularly to maintain compliance with CMS regulations and to ensure that they are providing the best possible care and services to their residents. The information gathered in this assessment is crucial for creating and updating individualized care plans for residents and for overall facility quality improvement. It is also subject to review by CMS surveyors during inspections to determine whether the nursing home is eligible for Medicare and Medicaid reimbursement.
